we appreciate, that you would like to participate in the german primary hyperoxaluria registry. This registry is open for all patients with either form of primary hyperoxaluria, whereever they reside and whereever they are treated. It is part of the OxalEurope PH-Registry.
For registration, please send an email to registration@ph-registry.net and please provide the following information Name, Title, Affiliation, phone, fax, email address. The information you have to provide for registration is necessary to ensure quality and authenticity of the data input. After registration, you will receive an email from us indicating your login details.
